In 1389 Khmilnyk passed to Prince Fedir Koriatovych, and in 1448 it received Magdeburg rights from Grand Duke Casimir IV Jagiellon. Crown Hetman Jan Tarnowski strengthened the castle in 1534, and from 1672 to 1699 the town was held by the Ottomans, who turned a corner tower into a mosque. In 1934, a search for drinking water uncovered radon springs, and Khmilnyk became a spa town.
Neurosurgeon Heorhiy Pedachenko (1923–2001) was born in Khmilnyk, and the Polish pianist, composer and statesman Ignacy Jan Paderewski (1860–1941) was born nearby in the village of Kurylivka. Writer and ethnographer Pelaheia Bartosh (1833–1904) lived here.
The town stands on the Southern Buh in northeastern Podillia. Its sights include the octagonal castle tower with a history museum, the unfinished Ksido Palace (1911–1915) by Ivan Fomin, the arched footbridge of 1915 and the Holy Trinity Church (1603), whose clock chimes every hour.
